Steps to Complete Your Accommodation Request

  1. Submit Required Documentation:To support your request, you must upload one of the following:
    • A doctor’s note detailing your need for an accommodation.
    • Documentation of a previously granted accommodation.
  2. Fill Out the Form:Please provide all necessary information in the form below. Ensure all fields are completed accurately to avoid any delays in processing your request.
  3. Await Confirmation:Once your form and documentation have been submitted, our team will review your request. You will be contacted regarding the acceptance or denial of your accommodation request.
  4. Next Steps if Approved:
    • Payment: Upon approval, you will need to proceed with the payment for the exam.
    • Receive Instructions: We will provide you with further instructions once we have completed setting up your accommodation.
    • Take the Exam: After receiving our confirmation, you may proceed with taking the PSHRA Certification Exam with the accommodation(s) in place.
